Certificate of Catholic Practice forms

CERTIFICATE OF CATHOLIC PRACTICE – SCHOOL APPLICATIONS 2026

For parents applying for a place at a Catholic school, a Certificate of Catholic Practice is required. The certificate is issued by the Parish Priest to parishioners who regularly attend Mass and are practising Catholics.

Fr Barto, our Parish Priest, will be available to sign certificates at two drop-in sessions in Our Lady Immaculate Parish Hall, from 5–7pm. There is no need to make an appointment, and Certificate of Catholic Practice forms will be available at the sessions.

Please remember to bring your child’s Baptism Certificate with you.

Secondary School Applications
